INSTRUCTIONS

  1. Each district elder should designate a person to collect and approve all of the photos.
  2. Do not use this form until all of the photos are collected for the district. 
  3. Submit the photos after paying for your licenses. 
  4. The photos should be renamed with the complete name and ID if one is available. 
  5. Pastors and Ministers should dress in Class B attire – see sample to the right. 
  6. All others should wear their appropriate vestments.
  7.  Submit high quality photos against solid white or beige wall.
  8. Do a head shot as shown to the right; not the full body. 
  9. Do not show any photos, plants, etc. in the background.  
  10. Only upload JPEG or PNG formats.
