INSTRUCTIONS
- Each district elder should designate a person to collect and approve all of the photos.
- Do not use this form until all of the photos are collected for the district.
- Submit the photos after paying for your licenses.
- The photos should be renamed with the complete name and ID if one is available.
- Pastors and Ministers should dress in Class B attire – see sample to the right.
- All others should wear their appropriate vestments.
- Submit high quality photos against solid white or beige wall.
- Do a head shot as shown to the right; not the full body.
- Do not show any photos, plants, etc in the background.
- Only upload jpeg on png formats.
